Our staff and volunteers are working very hard to continue to provide essential services to Haringey residents aged over 18 years old to settle home safely from hospital on discharge.

Our service provides practical and emotional support as follows:

• Assisting patients on day of discharge
• Helping patients with essential food shopping
• Collecting prescriptions
• Assisting patients to pay bills
• Providing telephone support
• Providing information and introduction to local services in the community to improve your health and wellbeing
