Are you passionate about providing older people with practical support and skills to live healthier and happier lives?

We are currently recruiting for a Home for Hospital Worker to be responsible for providing practical and emotional support to patients receiving the Home from Hospital service.

You will provide home accompaniment and home visits to Haringey residents aged over 18 years old leaving hospital for a period of four weeks to ensure safe discharge, promote independence and reduce the risk of re-admissions.
